This notice describes how Eudaven intends to handle protected health information (PHI) once clinical services begin.
Pre-launch scope
Eudaven is not yet operating as a covered entity. This site does not collect PHI — the waitlist gathers only contact and interest details, not health information. No HIPAA-covered relationship exists yet.
At launch
When clinical services open, Eudaven and its partners will handle PHI under HIPAA, including the Privacy and Security Rules. A full Notice of Privacy Practices will be provided before any health information is collected.
